At 11:30 a.m. Thursday, Senior Trooper Stinson responded to a two-vehicle fatal crash on US 421 north of County Road 525 North.
A preliminary investigation by Senior Trooper Stinson revealed that a red 2016 Buick Enclave, driven by Celestino Martinez, 80, of Monticello, was traveling northbound on US 421 and went left of the centerline for an unknown reason. At that time, a 2021 Ford IU Hospital Ambulance was traveling southbound on US 421 and was struck head-on by the Buick. Both vehicles came to rest on the east side of the road.
Martinez succumbed to his injuries and was pronounced deceased at the scene of the crash by the Carroll County Coroner. Notification to his family has been made.
The driver and passengers of the ambulance were transported by EMS to a Lafayette area hospital with non-life-threatening injuries.
